Medical organizational analysis of cancer morbidity, prevalence and mortality (Udmurt Republic data based)

Abstract

Introduction. Cases of Cancer have proved to be the leading causes of morbidity and mortality and determine the need for medical organizational analysis in order to improve public health system. Aim — to make a medical organizational analysis of cancer morbidity, prevalence and mortality in Udmurt Republic (UR). Materials and methods. The object of the research is the healthcare system of the UR (2002–2022). Data taken from from Rosstat (RF) and the Medical Information and Analytical Center of the Ministry of Health of the UR (2002–2022) were analyzed. The dynamics of cancer morbidity, prevalence and mortality (per 100 thousand population) in the UR and the RF were concidered. Vectors for improving of public health care for patients with cancer have been identified. Research methods: literature analysis, analytical, statistical, time series, comparative. Results and discussion. In 2002–2022, the dynamics of growth of cancer morbidity, prevalence and mortality and a shortage of oncologists in the Udmurt Republic were determined. The cancer morbidity increased from 260.5 to 486.6 per 100 thousand population (growth rate 186.8%, rate of increase 86.8%), more significantly among the urban compared to the rural population — positive growth of 60.5% and 49.5%, respectively; the cancer prevalence increased from 1296.5 to 2839.8 per 100 thousand population (growth rate 223.2%, rate of increase 123.2%); the proportion of patients with stage IV malignant neoplasms decreased in 2002–2012 and 2020–2022 to 21.5% and 23.8%, respectively, and increased in 2002–2020 to 25.8%; cancer mortality increased from 160.8 to 192.0 per 100 thousand population (growth rate 119,4%, rate of increase 19,4%), more significantly among the rural than among the urban population — the positive growth was 32.8% and 15.7%, respectively. The provision of oncologists in the Udmurt Republic in 2022 was 0.78 per 10 thousand population. Conclusions. The growth of cancer morbidity, prevalence and mortality and shortage of oncologists in the UR in 2012–2022 defined the need of improvement of oncological care by means of introduction of effective clinical organizational management models based on shift and telemedicine technology.
