Abstract

Introduction. The project “An integrated approach to improving the quality of life of the low-mobility population in the work of district nurses” was developed and implemented on the basis of the St. Petersburg State Medical University “City Polyclinic N 87”. Within the framework of the project, the problems of a patient with limited mobility were identified in order to work out a further plan for examination, monitoring and treatment together with a doctor in charge, concerning the realization of the basic needs of patients, using an integrated individual approach. The purpose of the study — to present the experience of the St. Petersburg city polyclinic in improving the effectiveness of medical care provided for people with limited mobility by optimizing nursing care in the conditions of the patient’s home. Materials and methods. Questionnaires have been developed for both the nursing staff and for patients with limited mobility to be applied before and in the process of the project. Secondary medical personnel underwent specialized training. The following results have been achieved during the events carried out. 1. District nurses have improved their knowledge of and skills in performing clinical examinations: 100% of district nurses make the rapid blood glucose test using a glucose meter; 91% are able to read electrocardiogram (ECG) results; 100% can assess respiration and frequency of respiratory movements (BDD). 2. Organization of care for a seriously ill patient: 100% of nurses will be able to identify the risk of pressure sores; to teach relatives how to use methods of caring for a seriously ill patient — 98%. 3. The amount of medical equipment being increased, problems with the ECG queue were eliminated. 4. The possibility of access to the apartments for providing patronage to a patient with limited mobility is gradually increasing. This is achieved as a result of personal calls to the patient or his relatives, aimed at making the appointment of the date and time of the nurse’s visit. 5. Information about the mobility of patients at the therapeutic site continues to be revealed, as a result of calls to the local district patients during the period of invitation for medical examination. Conclusion. The list of low-mobility patients living in district therapeutic areas has been updated. At the beginning of the project, a total of 489 patients with limited mobility were registered, as a result of the work carried out, 215 more patients were identified. 704 patients receive patronage care. The increase in coverage was fixed as 30.6%.
